The reveal at the end of Terry Gilliam's Brazil - the happy ending is a figment of imagination of the (driven insane by torture) hero. Also the final image in 28 Days Later - the woman and the girl walking down a hospital corridor reveals that the happy ending was just the final dying thoughts of Cillian Murphy's character. Oh and most of Jacob's Ladder of course.

Very obvious one, this, and surprised it's not in the top 10 list - The Vanishing (obviously the original, not the remake). But did you expect Mr Lucier to actually spoil as well as cite the spoiler?
